A meta-analysis, considering both common and random effects, was conducted after the exclusion process. Eight studies comprised the meta-analysis, involving a combined patient population of 15,140 individuals. This included 7,542 patients who were given propofol and 7,598 patients treated with volatile anesthetics. A mixed-effects model demonstrated that propofol anesthesia was linked to a lower incidence of postoperative acute kidney injury (AKI) compared to volatile anesthesia, with respective odds ratios of 0.63 (95% confidence interval 0.56-0.72) and 0.49 (95% confidence interval 0.33-0.73). The global health concern of Chronic Kidney Disease (CKD) of uncertain etiology (CKDu) disproportionately impacts tropical farming communities. The association between CKDu and environmental factors is substantial, diverging from the typical risk factors, like diabetes. Our study, the first to compare urinary proteomes in patients with CKDu and healthy controls from Sri Lanka, explores potential clues to disease etiology and diagnosis. Our study uncovered 944 proteins displaying differing abundance. In silico analysis yielded 636 proteins possessing a likely connection to kidney and urogenital structures. The presence of renal tubular injury in patients with CKDu, as expected, was substantiated by the increases in albumin, cystatin C, and 2-microglobulin. Interestingly, although some proteins, such as osteopontin and -N-acetylglucosaminidase, are usually increased in chronic kidney disease, a decrease was observed in patients with chronic kidney disease of unknown cause. Furthermore, the kidneys' expulsion of aquaporins, more prevalent in chronic kidney disease, was diminished in chronic kidney disease of unknown cause. Comparisons of CKDu's urinary proteome with prior CKD urinary proteome datasets revealed a distinctive and unique pattern. From our findings, there are potential early markers for diagnosing and distinguishing CKDu. Further studies are necessary to examine the role of lysosomal, mitochondrial, and protein reabsorption processes, and their interaction with the complement system and lipid metabolism in initiating and progressing CKDu. In cases where typical risk factors such as diabetes and hypertension are absent, and where molecular markers are lacking, discovering early disease indicators is vital.
